REGISTRATION POLICY REGISTRATIONS BY TELEPHONE, FAX OR ON-LINE

Registrations are not guaranteed until payment has been received.

 

PAYMENT OPTIONS BY ON-LINE: PRECISION GOALIE INSTITUTE OFFERS 3 PAYMENT OPTIONS AS LISTED BELOW

• 100% Full Payment at time of registration.

• 50% deposit at time of registration w/ 50% pre-authorized balance payment due on or before June 1st.

• Monthly payments to a maximum of 4 accepted with Credit Card or Posted Dated Cheques.

 

Upon receipt of registration, a deposit must be made to secure spot program and balance of payments will be spread over the remainder of the plan to a maximum of 4 months. Last payment to be processed on or before June 1st. Monthly payment processing dates are available on 1st, and 15th of the month.

 

RETURNED CHEQUES: Dishonoured cheques subject to a $25.00 NSF charge.

REFUNDS and CANCELLATIONS In the event a student cancels prior to June 1st, there will be a 10% administration fee charged for EACH session cancelled. No refunds will be made after June 1st.

 

BALANCE OF FEES All fees must be paid on or before June 1st. Late enrolments after June 1st. will require full payment at time of registration. Students will not be permitted on the ice without final payment. Should you choose the partial payment at time of registration and pay by credit card, a future payment will be schedule on the card provided on or before June 1st and an email confirmation will be sent for your information.

 

CONDITIONS OF ENROLMENT Precision Goalie Institute reserves the right to move students if they feel their choice of program is not within the evaluation guidelines dictated. This program adjustment will be made with the STUDENT’S best interest in mind.

 

STUDENT CONDUCT Precision Goalie Institute reserves the right to remove a student if behaviour has become disruptive to fellow students and the quality of the program. Precision Goalie Institute strongly enforces a zero-tolerance policy towards bullying. Student(s) participating in bullying will be asked to leave and no refund will be issued.

 

JERSEY Each participant will receive one goalie cut jersey at their summer camp check-in.

WHEN TO REGISTER We encourage you to register your goalies as soon as possible. We register all of our students on a first come first served bases.

ACKNOWLEDGING that Goaltending (ice hockey) is a sport, I agree that PRECISION GOALIE INSTITUTE, its agents, servants and employees shall not be liable to ME (the registered student) for any injury or damage resulting directly or indirectly from my participation at PRECISION GOALIE INSTITUTE, whether from my participation on ice warming up, on ice drills, on ice goaltending instruction, or off-ice dry land training and any other programs whether incurred on the ice or otherwise in or about the facilities. I further agree that I discharge PRECISION GOALIE INSTITUTE, its agents, servants and employees from all actions, claims and demands I may have for any injury or damage. I understand this agreement shall bind my heirs and assigns and shall inure to the benefit of the PRECISION GOALIE INSTITUTE, its agents, servants and employees and their successors and assigns. It is further agreed PRECISION GOALIE INSTITUTE does not and shall not be considered to guarantee or warrant such equipment as may be used in the conducting of its camps. PRECISION GOALIE INSTITUTE is not responsible for lost or stolen personal articles or equipment. PRECISION GOALIE INSTITUTE reserves the right to use any pictures taken during the school for advertising and/or instructional purpose.

 

REFUNDS will be honoured prior to JUNE 1ST. 10% administration fee will apply to all refunds or cancelled sessions No refunds will be made after June 1st. Any request for camp credits will have to be done so in writing and will be considered solely at the discretion of the camp regional office or for those covered under the Camp Refund Option (5% fee applicable). INCLUDED IS A DEPOSIT AND/OR FULL PAYMENT AS INDICATED ABOVE. I understand and agree the balance of these fees must be paid in full on or before June 1st. All registrations must be paid in full prior camp check-in. The outstanding balance may result in a student being withheld from first ice session. Late payment accepted will be by cash only. Cheques should be made payable to PRECISION GOALIE INSTITUTE. Registrations being FAXED or MAILED in must be acknowledged and signed by a parent or guardian before registration can be authorized by PRECISION GOALIE INSTITUTE. The submittance of the ONLINE registration acknowledges that the WAIVER CLAIM has been fully read and authorized by a parent or guardian responsible for the named student according to the Policies and Procedures.
